Equipment: Customer premise equipment is required and can be purchased from MV or provided by MV at an additional monthly rate -- in these cases, MV will perform the configuration of this equipment so that it is ready to plug in and go. See CPE description for information on the equipment that can be obtained through MV. Alternatively, you can provide your own site equipment, in which case MV may not be able to assist with setup nor guarantee operability.

Circuit cost: As noted above, T1 service relies on underlying Verizon circuits. Circuit pricing depends on several factors including distance, and may change over time as a result of Verizon's tariff changes, and changes in state and local fees. Your bill T1 bill may be adjusted to reflect such changes.

Circuit installation: MV will arrange the installation and maintenance of the Verizon line, and will bill you for the Verizon component. Unless you specifically want to deal with Verizon, your only maintenance and billing contact will be MV.

Standard Usage: For the standard usage pricing:

• You must be an end-user (and not resell access or otherwise provide access to third parties);

• The average daily bandwidth used on your circuit, either inbound or outbound, must not exceed 30% of the rated capacity more than 5 non-overlapping 12-hour periods during a calendar month.
